ABSTRACT

Introducción: La enfermedad inflamatoria pélvica aguda es una entidad frecuente en mujeres jóvenes en edad reproductiva, y constituye la causa principal de infertilidad. La búsqueda de un tratamiento antiinflamatorio eficaz y seguro que alivie el dolor, evite complicaciones y secuelas, es una prioridad para su tratamiento. Objetivo: Evaluar la eficacia y seguridad del tratamiento con Proctokinasa® en la enfermedad inflamatoria pélvica aguda grados I y II. Materiales y métodos: Se desarrolló un ensayo clínico, controlado, abierto, de enero de 2017 a enero de 2018, que incluyó 76 pacientes con diagnóstico de enfermedad inflamatoria pélvica aguda en la atención primaria de salud, quienes fueron asignadas aleatoriamente a dos grupos de tratamiento: grupo principal Proctokinasa®, que recibió una unidad/8 h por 2 días, y el grupo Piroxicam, un supositorio de 20 mg/24 h por 7 días. La antibioticoterapia oral recomendada se inició simultáneamente. Se evaluó la respuesta clínica y ecográfica al tratamiento. Los eventos adversos fueron evaluados. Resultados: Se constató respuesta clínica a partir de las 72 horas de tratamiento en más del 50 % de las pacientes en ambos grupos, y fue del 100 % de los casos para el grupo Proctokinasa® en las dos últimas evaluaciones realizadas (días 10 y 15). Se evidenció la remisión de las alteraciones ecográficas en el 96,9 % del grupo Proctokinasa®. Los eventos adversos fueron escasos y leves. Conclusiones: El empleo de Proctokinasa® resultó eficaz y seguro en el tratamiento de la enfermedad inflamatoria pélvica aguda grados I y II.


Introduction: Acute pelvic inflammatory disease is a common entity in young women of reproductive age and it is a leading cause of infertility. The search for an effective and safe anti-inflammatory treatment that relieves pain, avoids complications and sequels, it is priority for its treatment. Objective: To evaluate the efficacy and safety of the treatment with Proctokinasa® in acute pelvic inflammatory disease grades I and II. Materials and methods: An open, controlled clinical trial was carried out from January 2017 to January 2018 which included 76 patients diagnosed with acute pelvic inflammatory disease in primary health care; they were randomize assigned to two treatment groups: Proctokinasa® main group, who received one unity/8h for 2 days, and the Piroxican group, 1 suppository of 20 mg/24h per 7 days. The recommended oral antibiotic therapy was started simultaneously. Clinical and ultrasound response to treatment were evaluated. Adverse events were evaluated. Results: Clinical response was stated after 72 hours of treatment inmore than 50% of the patients in both groups, and it was 100% for the Proctokinasa® group in the last two evaluations (days 10 and 15). Remission of ultrasound alterations was evident in 96.9% of the Proctokinasa® group. Adverse events were few and mild. Conclusions: The use of Proctokinasa® was effective and safe in the treatment of acute pelvic inflammatory disease grades I and II.

Medicinal insects are good at running and scurrying, with the effectiveness of activating blood circulation and dispelling blood stasis, moving qi and relieving pain, searching and dredging collaterals, attacking hardness and dissipating mass, and purging foetus and resolving mass, etc. Appropriate prescriptions can enhance the effectiveness of the formula in eliminating blood stasis and eliminating stagnation. In the treatment of menstrual disorders, chronic pelvic inflammatory disease, uterine cavity disease, incomplete miscarriage, ectopic pregnancy, and other gynaecological diseases, medicinal insects including Quanxie (Scorpio), Wugong (Scolopendra), Tubiechong (Eupolyphaga/Steleophaga), Jiuxiangchong (Coridius chinenses), Shuizhi (Hirudo), Mangchong (Tabanus), Dilong (Pheretima) and other insects for medicinal purposes could be used, and the self-prescribed empirical formulas such as Sanhuang Decoction (三黄汤), Hongteng Decoction (红藤汤), Penning Decoction (盆宁方), Shapei Decoction (杀胚方), and Gongwaiyun Decoction (宫外孕方) and so on, were all applied medicinal insects and showed effective in clinic.

Resumen El diagnóstico diferencial de enfermedad inflamatoria pélvica (EIP) representa un desafío, porque tiene un gran polimorfismo en su forma de presentación y, de ser sintomática, se confunde con numerosas patologías que ocasionan dolor abdominal agudo. Neisseria gonorrhoeae es uno de sus agentes etiológicos más frecuentes. Se presenta el caso de una paciente de sexo femenino con síndrome de abdomen agudo asociado a vómitos. Los estudios de imágenes (ecografía y tomografía axial computada) revelaron la presencia de líquido interasas y apéndice aumentado de tamaño. Se decidió conducta quirúrgica. Durante la misma se tomó muestra de líquido abdominal de cuyo análisis microbiológico se recuperó N. gonorrhoeae, sensible a ceftriaxona y a azitromicina. La paciente fue tratada con estos antibióticos en dosis de 1 g/día/endovenoso, con buena evolución clínica. Frente a una paciente en edad fértil con abdomen agudo es útil recordar, por sus implicancias, la posibilidad de una EIP por N. gonorrhoeae.


Abstract Differential diagnosis of pelvic inflammatory disease (PID) represents a challenge because it has a great polymorphism in its presentations and, if symptomatic, there are numerous pathologies that cause acute abdominal pain. Neisseria gonorrhoeae is one of the most frequent etiological agents. The case of a female patient with acute abdominal syndrome associated with vomiting is presented. Imaging studies (ultrasound and computed tomography) revealed the presence of fluid between the intestinal loops and an enlarged appendix. Surgical management was decided, during which a sample of abdominal liquid was taken and N. gonorrhoeae, susceptible to ceftriaxone and azithromycin, was recovered from its microbiological analysis. The patient was treated with these antibiotics at doses of 1 g/day/iv, with good clinical evolution. In the case of a patient of childbearing age suffering from acute abdomen, it is useful to remember, because of its implications, the possibility of PID due to N. gonorrhoeae.


Resumo O diagnóstico diferencial de doença inflamatória pélvica (DIP), representa um desafio, porque tem um amplo polimorfismo em sua forma de apresentação e, caso seja sintomática, confunde-se com numerosas patologias que causam dor abdominal aguda. Neisseria gonorrhoeae é um dos seus agentes etiológicos mais frequentes. É apresentado o caso de uma paciente de sexo feminino com síndrome de abdome agudo associado com vômitos. Os estudos das imagens (ultrassonografia e tomografía axial computadorizada) revelaram a existência de líquido ascítico e apêndice aumentado de tamanho. Decidiu-se o tratamento cirúrgico. Durante a prática cirúrgica foi obtida amostra do líquido abdominal de cuja análise microbiológica foi recuperada N. gonorrhoeae, sensível à ceftriaxona e à azitromicina. A paciente recebeu o tratamento com esses antibióticos em dose de 1 g/dia/intravenosa, obtendo boa evolução clínica. Diante de uma paciente em idade fértil com abdome agudo, é útil lembrar, pelas suas consequências, a possibilidade de uma DIP por N. gonorrhoeae.

ABSTRACT

Background: Cervical discharge as part of cervicitis and pelvic inflammatory disease is a cause of significant morbidity in sexually active women worldwide. Non-gonococcal and non- chlamydial bacterial pathogens are becoming more prevalent. Aims: This study aims to determine bacterial pathogens causing cervical discharge using culture and/or polymerase chain reaction and assess the clinical and laboratory response to the conventional syndromic kit regimen established by the World Health Organisation. Methods: A retrospective review of records of women with cervical discharge over one year period. Culture and/or polymerase chain reaction results of endocervical swabs of various bacterial pathogens at baseline and after four weeks of treatment with syndromic kit regimen were recorded. Results: A total of 70 case records were reviewed for clinical details, out of which results of bacterial culture and polymerase chain reaction were available for 67 cases. Infectious aetiology was found in 30 (44.7%) patients with Ureaplasma species being the most common organism isolated on culture (18, 26.8%) and polymerase chain reaction (25, 37.3%), respectively. Polymerase chain reaction for Chlamydia trachomatis and Mycoplasma hominis was positive in ten (14.9%) and four (6%) cases, respectively. None of the patients showed positive culture for Neisseria gonorrhoeae. Coinfection was seen in eight (11.9%) patients with the majority showing Chlamydia trachomatis and Ureaplasma spp. coinfection (five patients). Forty one cases (58.5%) received tab. cefixime 400 mg and tab. azithromycin one gram stat (kit 1), while 29 cases (43.3%) received tab. cefixime 400 mg stat, tab. metronidazole 400 mg and cap. doxycycline 100 mg, both twice daily for 14 days (kit 6). Minimal to no clinical improvement with treatment was seen in 14 out of 32 cases (44%) at the end of four weeks with the conventional kit regimen. Post-treatment culture and/or polymerase chain reaction were positive in nine out of 28 cases (32.1%) with Ureaplasma spp. being the most common. Limitations: Retrospective study design, small sample size and fewer cases with follow-up data were the main limitations. Conclusion: Ureaplasma spp. was the most common infectious cause of cervical discharge in our patients. Treatment given as part of syndromic management led to a clinical and microbiological response in around half and two-third cases, respectively.

Aims@#This study was designed to evaluate awareness and knowledge of pelvic inflammatory disease (PID), its risk factors and diagnostic procedures among female undergraduates in tertiary institutions in Rivers State, Nigeria.@*Methodology and results@#A questionnaire-based survey design was conducted among 325 undergraduate students. Completed questionnaires were retrieved immediately. Descriptive and inferential (chi-square test) statistical tools were used for data analysis. Out of the 325 respondents, 186(57.2%) had heard of PID, 162(49.8%) did not know the possible risk factors of PID, 161(49.5%) perceived their awareness level of PID to be poor, while 30(9.2%) had good awareness level of PID. The majority 185(56.9%) of the respondents, do not know the mode of transmission of PID. There was a statistically significant relationship between the student’s age and the level of awareness statement at (p<0.05). On the student’s knowledge, only the statement: Do you know PID could be symptomless, showed a statistically significant relationship with the student’s course of study (χ2=12.815, p=0.00).@*Conclusion, significance and impact of study@#Most respondents have heard of PID via social media and seminars and have even seen those who had the disease. They still claim that their awareness level was poor since they do not know the mode of transmission and ill effects of PID, and so they cannot be protected against the disease. A sensitization campaign on risk factors, symptoms and mode of spread of the disease has to be carried out in the institutions to save the students at high risk of the infection.

ABSTRACT

@#【Objective】  To predict the active components and action targets of Wuyao (Linderae Radix) in the treatment of chronic pelvic inflammatory disease (CPID) based on network pharmacology, explore possible mechanisms of the treatment through animal experiments, and provide a scientific basis for clinical applications of Wuyao (Linderae Radix). 【Methods】  Possible active components and targets of Wuyao (Linderae Radix) in the treatment of CPID were obtained applying network pharmacology and molecular docking technology. CPID rat models were established using the mixed Escherichia coli, Staphylococcus aureus, and Ureaplasma urealyticum plus the performance of mechanical injury. Hematoxylineosin (HE) staining was applied to observe the pathological changes in the uterus, fallopian tube, and spleens of rat models. The contents of nitric oxide (NO), superoxide dismutase (SOD), and malondialdehyde (MDA) in the serum of rats were determined with the use of corresponding detection kits. Enzyme-linked immunosorbent assay (ELISA) test was used to measure the expression of interleukin (IL)-6 and IL-10 in the serum of rat models. Flow cytometry was used to determine the percentage of CD4+ and CD8a+ T cells as well as CD4+ CD25+ regulatory T cells (Tregs) in the spleen of rat models. 【Results】  A total of nine potential active components and four core therapeutic targets related to inflammatory response in Wuyao (Linderae Radix) were obtained. The animal experiments showed that Wuyao (Linderae Radix) markedly inhibited uterus swelling, regulated morphological changes in the fallopian tube and spleen,  effectively reduced inflammatory infiltration and injuries in the uterus and fallopian tube, and improved spleen functions in CPID rats. Moreover, Wuyao (Linderae Radix) markedly reduced the levels of NO, IL-6, and MDA, and increased the levels of IL-10 and SOD in the serum of rats. Wuyao (Linderae Radix) also elevated the percentage of CD4+T cells and the CD4+ T/CD8a+ T cell ratio, reduced the percentage of CD8a+ T cells, and raised the percentage of CD4+ CD25+ Tregs that had been abnormally decreased in rat models (P < 0.05). 【Conclusion】  Wuyao (Linderae Radix) could have therapeutic effects on CPID rats by relieving oxidative stress, mitigating inflammatory levels, and regulating the immuno-function of T cell subgroups to improve the pathological changes in CPID rats. It is a medicinal herb worth being further explored for its clinical values.

ABSTRACT

Pelvic inflammatory disease (PID) is an infection of women's reproductive organs, which lasts a long time and features frequent recurrence. It is mainly caused by apoptosis, low immunity, abnormal metabolism and blood rheology indexes, and bacterial imbalance. The pathological manifestations are tissue adhesion, fibrosis, and chronic inflammation, and inflammatory response occurs in the whole process of this disease. Therefore, interfering with the inflammatory response tends to be a solution. A few therapies are available in western medicine and antibiotics are commonly used. However, antibiotic resistance is still a bottleneck in the treatment and thus the symptoms of patients fail to be obviously relieved, with pelvic tissue adhesions remained. In the treatment of inflammation, traditional Chinese medicine has remarkable effect and internal-use and external-use medicines are both used, such as oral Chinese medicinal decoction, Chinese medicine enema, Chinese medicine iontophoresis, and acupuncture. They exert therapeutic effect by regulating the conduction of related signaling pathways and influencing the expression of inflammatory cytokines. At the moment, most articles focus on the efficacy of traditional Chinese medicine in the treatment of PID, and there is a lack of summary on traditional Chinese medicine regulation of relevant signaling pathways and the pathogenesis of PID. Therefore, by summarizing relevant research, this review proposed five signaling pathways related to the occurrence of this disease, namely, transforming growth factor-β/Smads protein (TGF-β/Smads) signaling pathway, Janus kinase/signal transducer and transcription activator (JAK/STAT) signaling pathway, nuclear factor-κB (NF-κB) signaling pathway, mitogen-activated protein kinase (MAPK) signaling pathway, and Hippo signaling pathway, and described the pathogenesis of this disease. Thereby, this study is expected to provide effective targets and ideas for the treatment of this disease.

Pelvic Inflammatory Disease (PID) is responsible for a considerable disease burden and represents an important healthcare issue in worldwide. PID is the clinical syndrome associated with upper genital tract infection and inflammation caused by the spread of micro-organisms from the lower to upper genital tract. Prevention of PID and recurrent PID is also a public health priority in the present scenario. Clinical signs and symptoms are relayed by clinician to diagnose PID, mainly due to the lack of a minimally or non-invasive diagnostic test that reliably identifies women with upper genital tract inflammation. Aim and objectives: The search for an Ayurveda equivalent of PID in Ayurveda classics doesn’t yield any one disease entity that entirely matches the clinical picture of this condition. Material and Methods: Diagnosis of PID is based on centers for disease control and prevention 2015 PID guideline. Considering it as ‘Anukta Vyadhi’ (unexplained disorder), this review attempts to understand the disorder by deriving its Vikaraprakriti (nature of disease), Adhishtanantarani (structures and sites affected), and Samutthanavishesha (specific etiological factor) based on a detailed analysis of the clinical features and other findings regarding the disease available in various textbooks and articles in the light of related references in Ayurvedic classics. Discussion and conclusion: PIDs are established in terms of Nidana Panchaka (five components of understanding a disease). Pittala and Paripluta Yonivyapad can be implied as pelvic inflammatory diseases

ABSTRACT

Objective:To investigate the effects of amoxicillin capsules combined with Fuke Qianjin Tablets on serum inflammatory factors in patients with acute pelvic inflammatory disease. Methods:A total of 240 patients with acute pelvic inflammatory disease who received treatment in Department of Obstetrics and Gynecology, Shizhu Branch of the First People's Hospital of Yongkang from January 2016 to June 2020 were included in this study. They were randomly assigned to receive either treatment with amoxicillin capsules and conventional drugs (control group, n = 120) or amoxicillin capsules, conventional drugs and Fuke Qianjin Tablets in combination (study group, n = 120). Serum inflammatory factors [C-reactive protein (CRP), interleukin-1 (IL-1), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α)] were compared between before and after treatment in each group and between two groups after treatment. Results:After treatment, CRP, IL-1, IL-6 and TNF-α levels in the control group were (34.28 ± 7.19) mg/L, (103.52 ± 30.18) mg/L, (103.27 ± 37.18) ng/L, and (64.11 ± 13.28) ng/L, respectively and they were (15.26 ± 6.49) mg/L, (60.44 ± 17.64) mg/L, (62.15 ± 15.33) ng/L, (38.19 ± 9.46) ng/L, respectively in the study group. Before treatment, CRP, IL-1, IL-6 and TNF-α levels in the control group were (89.32 ± 17.36) mg/L, (193.27 ± 52.18) mg/L, (180.48 ± 49.25) ng/L, (81.27 ± 18.16) ng/L , respectively and they were (90.48 ± 19.73) mg/L, (192.55 ± 51.84) mg/L, (178.93 ± 50.11) ng/L, (79.83 ± 17.35) ng/L, respectively in the study group. CRP, IL-1, IL-6 and TNF-α levels post-treatment were significantly lower than the levels pre-treatment in each group (control group: tCRP = 11.28, P < 0.001; tIL-1 = 12.73, P < 0.001; tIL-6 = 9.48, P < 0.001; tTNF-α = 8.94, P < 0.001; study group: tCRP = 12.38, P < 0.001; tIL-1 = 18.44, P < 0.001; tIL-6 = 13.29, P < 0 .001; tTNF-α = 12.43, P < 0.001). CRP, IL-1, IL-6 and TNF-α levels post-treatment in the study group were significantly lower than those in the control group ( tCRP = 9.37, P < 0.001; tIL-1 = 11.84, P < 0.001; tIL-6 = 8.33, P < 0.001; tTNF-α = 7.38, P = 0.002). Response rate in the study group was significantly higher than that in the control group (95.83% vs. 75.00%, χ2 = 27.29, P < 0.001). Before and after treatment, there were no significant differences in safety indicators between the two groups (all P > 0.05). Conclusion:Amoxicillin capsules combined with Fuke Qianjin Tablets is highly effective on acute pelvic inflammatory disease, greatly lower inflammatory factor levels, and is highly safe.

RESUMEN Introducción: la apendicitis aguda es causa frecuente de dolor abdominal. Su complicación más frecuente es la infección del sitio operatorio, siendo el compromiso pélvico muy raro. Presentamos el caso de una paciente sin vida sexual activa, con una enfermedad pélvica inflamatoria como complicación posoperatoria de una apendicitis aguda. Esta ingresó por dolor abdominal, varias semanas después de una apendicetomía, sin signos de infección incisional, pero con una tomografía que reportaba un hidrosalpinx izquierdo. Evolucionó hacia el deterioro clínico a pesar de la reanimación con cristaloides y antibióticos de amplio espectro. Fue llevada a laparotomía para control del foco. Se encontró un hidrosalpinx izquierdo y necesitó de una segunda cirugía por el compromiso anexial derecho. Luego de estas intervenciones hubo recuperación. Conclusión: la enfermedad pélvica inflamatoria es una etiología poco frecuente (aunque posible) de abdomen agudo en pacientes sin vida sexual activa.


SUMMARY Introduction: Acute appendicitis is a frequent cause of abdominal pain and its most frequent complication is surgical site infection, with pelvic involvement being very rare. The following is a case of a patient with pelvic inflammatory disease as postoperative complication after appendectomy. The patient was admitted for abdominal pain several weeks after an appendectomy, without signs of incisional surgical site infection but with a CT scan that re-ported a left hydrosalpinx. The patient progresses with clinical deterioration despite resuscitation with crystalloids and broad-spectrum antibiotics. A laparotomy was performed for infection control. A left hydrosalpinx was found and needed a second surgery due to right adnexal involvement. After this she recovered. Conclusion: Pelvic inflammatory disease is a rare but possible etiology of the acute abdomen in patients who's never had sexual intercourse.

RESUMEN Introducción: La enfermedad inflamatoria pélvica es la infección grave más frecuente en mujeres entre 16 y 25 años. La adolescencia es el periodo de mayor riesgo de aparición por la mayor incidencia de factores de riesgo relacionados con conductas sexuales inseguras. Objetivo: Determinar las características clínico epidemiológicas de las pacientes con diagnóstico de enfermedad inflamatoria pélvica tumoral. Método: Se trabajó con una población de 63 pacientes. Se conformaron dos grupos de estudio, grupo I con las adolescentes y jóvenes, y grupo II con las demás pacientes. Las variables utilizadas fueron: relaciones sexuales precoces, número de parejas sexuales, relaciones sexuales desprotegidas, uso de dispositivos intrauterinos, antecedentes de interrupción de embarazo, principales manifestaciones clínicas, resultados de complementarios y tratamiento administrado. Resultados: Se encontró que el 76,4 % de las adolescentes tenían relaciones sexuales desprotegidas, 52,9 % comenzaron las relaciones sexuales antes de los 14 años, 64,7 % refirieron antecedentes de abortos provocados, y el 100 % solicitó atención por dolor pélvico. En el 95,6 % de las pacientes se utilizó tratamiento médico. Conclusiones: En la mayoría de los casos, el cuadro clínico y los complementarios realizados fueron consistentes con la enfermedad inflamatoria pélvica tumoral; y el tratamiento utilizado fue médico.


ABSTRACT Introduction: Pelvic inflammatory disease is the most frequent serious infection in women between 16 and 25 years old. Adolescence is the period of highest risk of onset due to the higher incidence of risk factors related to unsafe sexual behaviors. Objective: To determine the clinical and epidemiological characteristics of patients diagnosed with tumoral pelvic inflammatory disease. Methods: We worked with a population of 63 patients. Two study groups were formed, group I with adolescents and young people, and group II with the other patients. The variables used were: early sexual intercourse, number of sexual partners, unprotected intercourse, use of intrauterine devices, history of pregnancy termination, main clinical manifestations, complementary results, and treatment administered. Results: It was found that 76.4% of the adolescents had unprotected sexual relations, 52.9% began sexual relations before the age of 14, 64.7% referred a history of induced abortions, and 100% requested attention for pain pelvic. Medical treatment was used in 95.6% of the patients. Conclusions: In most of the cases, the clinical and complementary symptoms performed were consistent with tumor pelvic inflammatory disease; and the treatment used was medical.

Este trabalho buscou reunir dados essenciais sobre as etiologias de dor pélvica aguda, uma queixa constante nos serviços de emergências e ambulatórios de ginecologia, responsável por grande desconforto e impacto na qualidade de vida de pacientes mulheres. É uma condição laboriosa por causa de seu amplo espectro de causas, devendo ser abordada com cuidado e atenção pelo profissional médico, o qual deve considerar os diversos diagnósticos diferenciais, sendo a ultrassonografia o exame de maior importância para auxiliar em seu diagnóstico. As principais etiologias não obstétricas podem ser não ginecológicas e ginecológicas; essas últimas são divididas em anexiais e uterinas. Entre as causas ginecológicas, devem- -se investigar cistos ovarianos, torções anexiais, leiomiomas, doença inflamatória pélvica, abscesso tubo-ovariano, dismenorreia e complicações de dispositivos intrauterinos. A maioria das causas tem tratamento eficaz, com retorno da função do órgão e melhora da qualidade de vida, sem complicações, especialmente se diagnosticada precocemente.(AU)


The aim of this study was to gather important data on acute pelvic pain etiologies, a usual complaint in the emergency services and gynecology outpatient clinics, responsible for great discomfort and impact on quality of life in female patients. It is a laborious condition due to its wide spectrum of causes, which needs to be approached with attention by the physician, who must consider all the possible diagnoses, being the ultrasonography the most important exam to detect it. The main non-obstetric etiologies can be non-gynecological and gynecological, which are separated in adnexal and uterine causes. Among the gynecological causes, ovarian cysts, adnexal torsions, leiomyomas, pelvic inflammatory disease, ovarian tube abscess, dysmenorrhea and complications of intrauterine devices should be investigated. Most causes can be effectively treated, with return of organ function and improved quality of life, without complications, especially if diagnosed early.(AU)

ABSTRACT

Pelvic inflammatory disease(PID) has become one of the leading causes of female infertility, with an increasing incidence in recent years. Modern medicine believes that risk factors of PID will affect the formation of eggs and embryo implantation, especially on the encounter of gametes, fertilization, and transport of fertilized eggs to the uterine cavity. Therapies for infertility due to PID include medication, sonographic hydrotubation, surgery, and assisted reproductive technology. Professor Ma Kun believes that the disease is located in the uterus with appendages with the main pathogenesis of kidney deficiency and blood stasis. Blood stasis is the pathological basis, and kidney deficiency is the fundamental pathogenesis, which exhibits deficiency-excess in complexity. Kidney deficiency will cause blood stasis over time, while blood stasis will aggravate kidney deficiency in turn, making PID-induced infertility refractory. In clinical practice, basic therapies follow the principles of kidney-tonifying and blood-activating, removing blood stasis, dredging collaterals, and coordinating thoroughfare and conception vessels. The oral administration of Chinese medicine combined with enema, external application, and external washing displays better efficacy in improving the pelvic microenvironment and increasing the pregnancy rate and pregnancy success rate in the treatment of PID-induced infertility.

ABSTRACT

OBJECTIVE@#To compare the therapeutic effect between acupuncture combined with ibuprofen sustained-release capsule and simple ibuprofen sustained-release capsule on chronic pelvic pain (CPP) after pelvic inflammatory disease (PID).@*METHODS@#A total of 144 patients were randomized into an observation group (72 cases, 10 cases dropped off) and a control group (72 cases, 9 cases dropped off). Ibuprofen sustained-release capsule was given orally in the control group, one capsule a time. On the basis of the treatment in the control group, acupuncture was applied at Guanyuan (CV 4), Shuidao (ST 28), Guilai (ST 29), Shenshu (BL 23) and Ciliao (BL 32), and Shuidao (ST 28), Guilai (ST 29), Shenshu (BL 23) and Ciliao (BL 32) were connected to electroacupuncture in the observation group. The treatment was given 10 days before menstruation, once a day for 3 menstrual cycles in both groups, and the follow-up was adopted 3 menstrual cycles after treatment. The visual analogue scale (VAS) scores of hypogastrium and lumbosacral region before treatment, after treatment, and at the follow-up, the score of local signs and the score of World Health Organization quality of life questionnaire-brief version (WHOQOL-BREF) before and after treatment were observed in the both groups.@*RESULTS@#After treatment and at the follow-up, the VAS scores of hypogastrium and lumbosacral region were decreased compared before treatment in both groups (@*CONCLUSION@#Acupuncture combined with ibuprofen sustained-release capsule can effectively improve the symptoms, signs and quality of life in patients with CPP after PID, the therapeutic effect is superior to simple ibuprofen sustained-release capsule.

OBJECTIVE@#To evaluate the effect and safety of Kangfuyan Capsules () for treating pelvic inflammatory disease (PID) in patients with chronic pelvic pain (CPP) in a multicenter, randomized, controlled, double-blind, parallel-group clinical trial.@*METHODS@#Totally, 240 PID patients with CPP were randomized into 2 groups using a computer generated random number at a 1:1 ratio from 10 hospitals in China between September 2014 and November 2015. Patients received either oral Kangfuyan Capsules or Gongyanping Capsules (, control); the regimen for both groups comprised 4 capsules (3 times daily) for 12 weeks, with follow-up visit 4 weeks after treatment. The visual analogue scale (VAS) scores, clinical responses, remarkable cure rates for each symptom, and quality of life scores were assessed at baseline, and after 1, 2, and 3 months. Adverse events were also recorded.@*RESULTS@#The VAS scores were significantly lower (P<0.05), whereas the clinical responses, remarkable cure rates for lower abdominal pain, uterine tenderness, adnexal mass, and adnexal tenderness, and Health-related quality of life (EQ-5D) scores were higher in the Kangfuyan group than in the control group at 3 months (P<0.05). Common treatment-related adverse events included high hepatic enzyme levels, reduced hemoglobin levels, and elevated platelet counts, although all the adverse events were either mild or moderate in severity.@*CONCLUSION@#Compared with Gongyanping therapy, Kangfuyan therapy yielded markedly better analgesia effects for CPP caused by PID, with obvious long-term efficacy and good safety. ABSTRACT

Background: Infertility is defined as one year of unprotected intercourse without pregnancy, primary in which no previous pregnancies have occurred and secondary in which a prior pregnancy not necessarily a live birth has occurred. The objective of the study was to compare the relative efficacy of hysterosalpingography (HSG) and laparoscopy with chromopertubation in the diagnosis of tubal factors in infertile women.Methods: 90 infertile women attending the infertility clinic at Dharmapuri Medical College and Hospital were selected for this study. The study period was from April 2016 to July 2017 these patients were initially counseled along with their partners and a thorough history of both the partners was obtained followed by a general and pelvic examination of female partners.Results: HSG results indicated that 48 patients had tubal pathology and 11 patients had uterine pathology. Of those 11 patients with uterine pathology, 6 patients with synechiae had both tubal and uterine pathology. The remaining 5 had only uterine pathology and tubes were patent in them. The site of tubal occlusion in all those blocked tubes shows that more number of the tubal blockage was seen in mid segment of the tube (24 cases) followed by the fimbrial block in 16 cases. Diagnostic laparoscopy with chromopertubation was performed in all 90 patients and the findings were recorded.Conclusions: HSG has reasonably good sensitivity and specificity in diagnosing tubal pathology of infertile women. But given the high rate of false-positive diagnosis of tubal pathology (29%) in HSG, a follow-up laparoscopy is warranted.

Background: Ectopic pregnancy (EP) is the leading cause of maternal morbidity and mortality in the first trimester and major cause of reduced reproductive potential. Early detection of EP by improved ultrasonography modalities has decreased the rate of rupture and consequent maternal morbidity. Aim was to study the predisposing risk factors in modern scenario and choose the appropriate management available.Methods: A retrospective study on clinical diagnosis and management of EP of patients was carried out between January 2018 to February 2020. Investigations included CBC, UPT, serum β-hCG and TVS. Management was decided after thorough evaluation.Results: Out of 7,780 deliveries, 70 were EP (0.9%). Women with age 21-30 year had highest incidence (85.7%). Common symptoms were abdominal pain (94%), amenorrhea (87%), bleeding per vagina (48%). Most common risk factor associated with EP was PID (28.5%). Tubal EP was most common (84.2%) involving ampulla (66%), isthmus (15%), fimbria (12%), interstitial (7%). Scar ectopic was reported in 10% of cases and ovarian, rudimentary horn and abdominal pregnancy in 1.4% each. About 52.8% of ectopic was ruptured and salpingectomy was done in 74.3% and salpingo-oophorectomy in 2.8%. Five cases of scar EP required hysterotomy and 1 case was managed by methotrexate (MTX).Conclusions: EP remains a major challenge to the obstetrician worldwide. A high index of suspicion is required for early diagnosis and timely intervention in the form of medical or surgical treatment will definitely help in reducing the morbidity and mortality.

Introducción: La infertilidad se define como el no logro del embarazo posterior a 12 meses de relaciones sexuales sin protección, según la Organización Mundial de la Salud (OMS). Entre 60 a 80 millones de parejas anualmente sufren este problema. En el Perú no existen reportes actualizados que permitan conocer estas cifras y hay pocos estudios que permitan conocer las causas de esta afección. Objetivo: Determinar los factores de riesgo asociados a infertilidad en pacientes mujeres atendidas en consultorios externos en el Servicio de Ginecología del Hospital Vitarte, periodo enero 2015 a abril del 2019. Métodos: Se realizó un estudio observacional, analítico, retrospectivo de casos y controles, considerando una muestra de 82 casos y 82 controles. Se consideraron factores de riesgo clínicos, demográficos y de hábitos de consumo nocivo y, para medir la asociación, se utilizó los odds ratio crudos y ajustados, para lo cual se usó el paquete estadístico SPSS. Resultados: En base a los OR ajustados, se identificó a la dispareunia (OR:4.16, IC95% 1.56- 11.14), dismenorrea, (OR:4.22, IC95% 1.79-9.93), enfermedad pélvica infamatoria (OR:5.59, IC95% 2.11- 14.84), y el consumo de alcohol (OR:2.87, IC95% 1.10-7.49), como factores de riesgo estadísticamente significativos para el desarrollo de infertilidad. Conclusión: Los factores asociados a la infertilidad en el servicio de ginecología del Hospital Vitarte durante el periodo de enero 2015 a abril 2019 son la dispareunia, dismenorrea, el antecedente de enfermedad pélvica inflamatoria y, el consumo de alcohol.

Between 60 to 80 million couples annually suffer from this problem. In Peru there are no updated reports that allow to know these figures and there are few studies that allow to know the causes of this condition Objective: To determine the risk factors associated with infertility in female patients treated in outpatient offices in the Gynecology Service of the Vitarte Hospital, period January 2015 to April 2019. Methods: An observational, analytical, retrospective case-control study was conducted, considering a sample of 82 cases and 82 controls. Clinical, demographic and habits of harmful consumption factors were considered and, to measure the association, the crude and adjusted odds ratios were used, for which the statistical package SPSS was used. Results: Based on the adjusted OR, dyspareunia was identified (OR: 4.16, 95% CI 1.56-11.14), dysmenorrhea, (OR: 4.22, 95% CI 1.79-9.93), pelvic inflammatory disease (OR: 5.59, 95% CI 2.11-14.84), and alcohol consumption (OR: 2.87, 95% CI 1.10-7.49), as statistically significant risk factors for the development of infertility. Conclusion: The factors associated with infertility in the gynecology service of the Vitarte Hospital during the period from January 2015 to April 2019 are dyspareunia, dysmenorrhea, the history of pelvic inflammatory disease and alcohol consumption.

ABSTRACT

The incidence of unilateral live twin ectopic pregnancy is approximately 1:1,25,000 but increased in last few decades due to increased use of fertility drugs for ovulation induction, superovulation and assisted reproductive technology. It is also associated with increased incidence of pelvic inflammatory diseases. Patient was 30 years old G3P2L2 who presented to OPD of this LLRM medical college, Meerut with complain of amenorrhoea of two and half months. Pelvic examination revealed tenderness in lower abdomen, left adnexal fullness and tenderness without any bleeding. On USG, left adnexal monochorionic monoamniotic ectopic twins with normal cardiac activity. Exploratory laparotomy followed by left sided salpingectomy was done and patient was discharged with satisfactory condition. This is a very rare case and PID is also among risk factors and may also cause twin ectopic pregnancy.
